MNsure

You may qualify for a special enrollment period (SEP) if you are released from incarceration. Applicants must send a copy their release documents from the department of corrections or correctional facility showing their name and release date. Documents must show that you were released from incarceration within the past 60 days. Community Action Partnership of Hennepin County (CAP-HC) offers In-person Assistance With MNSure applications.

CAP-HC has partnered with agencies throughout the Hennepin County to offer on-site MNsure application assistance to apply for or renew your health insurance benefits. Visit their website to access the most recent clinic schedule.

Social Security Number for each person applying
Date of birth for everyone in household
Driver's license, Tribal ID and/or other ID
For non-citizens, green card or other immigration documents
Most recent year’s tax forms (1040EZ or page 1 of 1040)
Two of your most recent pay stubs (if applicable)
Documents for other sources of income
If you have an existing MNsure account, bring your username and password
Information about any employer provided health insurance available to each person applying (request this from employer)
